Please complete the Claim Submission form and include it with all loss documentation including insurance policies with declaration pages, repair estimates, contents inventories, photographs, police reports, etc.
Homeowner / Business Owner Submission Form
Your claim can be submitted to CDR by mail, fax, or e-mail. Please keep in mind, larger submissions should be mailed directly to our office at 1100 Dexter Avenue North, Seattle, WA 98109.
CDR charges a Claim Submission Flat Fee of $100. This is a preliminary charge wherein CDR determines if there is enough information and adequate justification to move forward on the claim, and which services we recommend to bring your claim to a satisfactory conclusion.
A CDR Representative will contact you upon receipt of your claim submission. Please make sure to include all of your contact information on the Claim Submission form.
